About Us
At Boulevard Terrace, we understand that health care is a people business. Our
dedicated caregivers are trained to treat all of our residents, their families,
and other staff members with respect. Our excellent staff-to-resident ratio
allows us to provide care on a more individualized basis, and creates an
atmosphere that enhances care by reducing employee turnover.
At Boulevard Terrace we provide a tradition of quality care with the most modern
rehabilitation and restorative nursing techniques. Established in 1969, our
facility was remodeled in 2005 as part of our continuing commitment to provide
the kind of clean, comfortable and attractive environment families expect.
We are a 100-bed facility with a spacious, enclosed courtyard that includes a
fountain and a shaded seating area. Our spacious and welcoming activity room is
a busy place for special events, and is also available to families for special
occasions, along with a private conference room.
We are a NO SMOKING facility.
As part of our effort to continually improve our services and employ “best
practices,” Boulevard Terrace participates in the Medicare Q-Source Quality
Initiative. We offer “companion radio,” a satellite-radio program designed for
our residents to enjoy and for staff members to receive special training.
Boulevard Terrace is locally owned and operated. For more information, contact
us at (615) 896-4505.
An Award-Winning Facility
Boulevard Terrace is a member of the
Tennessee Health Care Association and the
American Health Care Association. Our facility has been honored a number of
times by THCA, including two Innovative Program Awards for our restorative
dining program and our intergenerational program. Several of our department
heads have received “Member of the Year” awards from their respective affiliate
groups within THCA.
